Do you have a family member or patient you feel would benefit from therapy but would have difficulties travelling to a hospital or outpatient clinic?
We offer a Domiciliary Neuro Therapy Service from the Hospital of St John & St Elizabeth, which is available to patients living within a 3 mile radius of St Johns Wood. Recent developments will allow us to dedicate more staff to Domiciliary Services and we aim to build this therapy, offering more patients the benefits of being assessed and treated in their own home.
A team of highly skilled senior physiotherapists, occupational therapists and speech and language therapists will work with patients to achieve their goals. Therapists will be able to identify difficulties patients may be experiencing within their home as well any issues with accessing the community environment. Through liaison with other multi-disciplinary teams the community-based therapists can facilitate a successful discharge from hospital or prevent admission or readmission.
Conditions suitable for referral include:
- Neurological conditions e.g. Stroke, Parkinson’s Disease
- Loss/decrease of mobility/function
- Fear or history of falls
Referrals can be made by Consultant or GP or through self referral. Referral should include full patient details including reason for referral, contact details, address and relevant medical history.
